A Message from the School Nurse. Attention to parent of Pre-K, Kindergarten, and newly transferred students. All students enrolled in NC Public School for the first time must submit a complete immunization record and the NC Health Assessment form within 30 calendar days of enrollment. The NC Health Assessment must be completed by a licensed health care provider and the date of examination can be no more than 1 year prior to the first date of attendance. If your child has already had their physical within the acceptable time frame, please contact your private provider and have the health assessment form filled out. The deadline for submitting this information is September 27, 2023. Failure to turn in the required information can result in exclusion from school.
